The Chicago Patient Safety Forum welcomes ALL Chicago area healthcare professionals, patient advocates, educators, students and insurers interested in facilitating innovative systematic approaches to understanding and improving patient safety in the Chicago metro region. The CPSF welcomes all contributions; however membership is FREE.

As a CPSF member, you and/or your organization will have the opportunity to collaborate with a national network of patient safety organizations and can advise on patient safety scientific/research projects and events, work on patient safety policy issues, and participate in CPSF professional development and networking.

MEMBERSHIP CATEGORIES

Council of Organizations

If you represent a healthcare association, institution, facility, provider group, government agency, educational institution, consumer organization, or community join the Council of Organizations. CPSF requests that your organization designate a single representative to represent your organization and serve on the Council of Organizations. NOTE: Additional individuals from your organization are welcome to join CPSF under one of the following categories. (Join Here).

Scientific Council

Representatives from the academic research community committed to promoting patient safety through evidenced-based science to further the adoption of systemic improvements in patient safety (Join Here).

Professional Council

All individuals working in healthcare fields (Join Here).

Patient/Consumer Council

All non-healthcare professionals and consumer organizations representing patient/consumer interests in patient safety (Join Here).

Friends of the Forum

Friends of the Forum Members are those persons willing to assist the CPSF in both building the influence of the CPSF and in raising funds. (Join Here).

RIGHTS OF MEMBERS

The Professional, Scientific, Patient Consumer and Organizational Members shall have the right to:

      Vote at meetings of the Members.

      Participate in the meetings of Membership to be held at least annually.

      Present agenda items to the Steering Committee for consideration.

      Serve on any standing committee or initiative/task force.
