Chicago Patient Safety Forum (CPSF) gathers and shares information from lessons learned in hospitals, private practice, pharmacies, rehabilitation centers and long-term health care facilities.
CPSF Mission:
The Chicago Patient Safety Forum is a community-based network whose purpose is to facilitate innovative system approaches to understanding and improving patient safety in the Chicago metropolitan area.
CPSF Goals
To promote a culture of safety in healthcare among all stakeholders in our community
Develop community leaders in patient safety
Coordinate inter-organizational efforts to reduce medical errors
Increase public awareness of patient safety issues

Our members are healthcare professionals, patients and patient advocates committed to:
- Reducing preventable medical errors
- Avoiding repeating those errors
- Improving the quality of healthcare
- Never stop learning
Free membership
CPSF welcomes Chicago area healthcare professionals, patient advocates, educators, students and insurers from the public and private sector. CPSF collaborates with a national network of patient safety organizations. There are no membership dues. Click here to join CPSF.
