2005 Patient Safety Learning Collaborative: Medication Reconciliation- The first Collaborative launched January 2005 was a partnership between the Chicago Patient Safety Forum and the Illinois Hospital Association. The 2005 Learning Collaborative on Medication Reconciliation consisted of 25 hospitals and one health system as participants. Hospitals are working hard to design a systemic process that develops an up-to-date, one-source medication profile for all caregivers to reference upon a patient's admission, transfer and discharge. Through this medication reconciliation process, patients' medications are verified to be appropriate. Any inconsistencies revealed by comparing the real-time profile against physician orders are brought to the attention of the physician and, if warranted, changes are made to the orders.
An Outcomes Congress to share the collective learning with other Illinois hospitals was held October 18, 2005 in Bloomington, Illinois.
